On Monday, December 8, 2025, to assist families in preparing for St. Tikhon's Summer Camp 2026 application period, the Diocese is releasing the required medical forms as "Early Bird Forms" ahead of time.
Please Note: Completing These Forms Early Does Not Guarantee a Spot at Summer Camp 2026
Parents and guardians, applications for Summer Camp 2026 will open on February 14. Last year, the medical clearance timeline created some stress for families, so the required forms are being made available early. Please complete and save the PDFs now so you are prepared when the application window opens.
A reminder: the physical examination must be dated within twelve months of camp. If your child received a physical this past summer or fall, no new exam is required. The physician who performed the examination may simply sign the camp form.
Provided below are the required Medical Examination Form for all campers and the supplemental medical forms for those with specific conditions. Beginning February 14, you will also be asked to upload an image or PDF of your insurance information.
